PURCHASE AGREEMENT & HEALTH GUARANTEE

This puppy was examined on by Dr. Thompson of Southgate Animal Hospital and found to be in good health, please see accompanying health record.  Additionally this puppy was vaccinated with DA2PP on , please see accompanying health records.

• The breeders guarantee this dog to be in good health to the best of our knowledge at the time of purchase.   

• Buyer shall have 14 days to have the puppy examined by a licensed veterinarian.   If the puppy is not found to be in good health, the breeder shall be contacted within three days with a letter from the veterinarian stating the problem.  The breeder will do everything possible to work out a solution that will please Buyer. 
• After 14 days there will be no warranty on infectious diseases or illnesses which can be treated with medications. 
• The breeder will provide a 2 year guarantee on any life threatening or debilitating congenital/hereditary or genetic diseases.  Diagnosis must be made by a licensed veterinarian and provided in writing to the breeder.  Replacement of puppy will be made as soon as one of the same value/sex becomes available.  This does not apply to faults or defects which were made known at the time of purchase.  CASH REFUNDS will only be made at the discretion of the breeder.
• We reserve the right to have the puppy examined by a veterinarian we choose at our expense for a second opinion.
• The breeder will not be held responsible for any vet bills incurred and are solely the responsibility of the buyer.
• This contract is between the ORIGINAL purchaser and breeder only. 

Registration Papers

• This puppy was born of registered parents (AKC), and is sold as a companion/pet with limited registration papers.
• This puppy WILL NOT be eligible for full AKC registration (so the puppy can be bred or shown) in the future.
• When this puppy has been altered (spayed or neutered) the owner can send a copy of the alter certificate to the breeder and the AKC registration papers will be sent to the owner.
• If this puppy is not altered by 1 year of age, this guarantee is null and void.
• If this dog reproduces, accidently or on purpose, the purchaser owes a penalty of $1000.00.

Buyer agrees to all terms and conditions and realizes that failure to follow said terms and conditions will void this agreement and guarantee.
